Patient and Family Advisory Council


The Patient and Family Advisory Council (PFAC) is a collaborative group comprised of patients, family members, and healthcare professionals who work together to improve the quality, safety, and experience of care. It plays a key role in ensuring that the voices of patients and their families are heard and integrated into Heart Hospital’s policies, services, and care delivery.​

 

PFAC’s Role at Heart Hospital


As an integral part of Heart Hospital’s Person-Centered Care strategy, PFAC plays a key role in shaping policies, improving services, and fostering a culture where patients and families are active, empowered partners in their care journey.

PFAC members contribute by:

  • Co-designing services with patients and families to ensure care aligns with their preferences and cultural values.
  • Advising hospital policies and initiatives that directly impact the patient and family experience.
  • Reviewing patient education materials, communication practices, and facility enhancements to improve accessibility and clarity.
  • Promoting empathy, respect, and collaboration between healthcare teams and the people they serve.

Through their ongoing contributions, PFAC helps ensure that every aspect of care at Heart Hospital is compassionate, person-centered, and responsive to the voices of patients and their loved ones.
